Our office participates with BCBS, Medicare,
SPHN, PPOM, Aetna, BCN, PHP and Worker's Compensation.
For all other insurance carriers, the patient
will be responsible for payment at the time of the
appointment. However, a claim will be submitted to the
insurance on the patient's behalf.
Those insurance carriers
that need authorization must have it done prior to
the appointment otherwise the appointment will be rescheduled.
If the condition is a result of an auto accident, written
authorization is needed prior to the appointment. Most
auto companies will
not give authorization however will supply in writing
a notice of an open claim with current billing information.
Copay
amounts are due at the appointment
unless other arrangements have been made with the office
manager in advance. The office accepts payment in the
form of cash, check, VISA or MasterCard.
If there is no insurance, please make arrangements
with the office manager. A financial agreement can
be arranged for payment.
It is the patient's responsibility
to check the insurance for participation, copay, deductible
and authorizations. Physicians participate with insurance
carriers to assist the patient; however,
a physician can drop out of a network. Therefore, it
is best to check with the office if there are any concerns. |
